New Patient Paperwork
If you have already scheduled your new patient appointment, please complete this new patient paperwork packet (new patient appointments must be scheduled first).

For prescriptions refills by phone call our office at (615) 893-4896.
This refill service applies to all current patients who have been seen recently by one of our Advanced Practice Providers.
Please leave a message with the following information:
- First and Last Name in our Records
- Date of birth
- Name of the Pharmacy if you’re requesting a medication that requires you to take a prescription with you to the pharmacy (e.g., stimulant medication). Please also indicate the date you would like to pick up the prescription, and it will be left for you at the front desk.
- If you must discuss the refill with one of our staff members, please leave a message requesting a callback.
Please allow up to 24 hours (during the work week) for a return call. If it’s urgent, please indicate so in your message. Our staff will call you back as soon as possible, however, we can’t guarantee a same-day response on prescription refills.
PLEASE NOTE: The Sleep Centers of Middle Tennessee reserves the right to require a follow-up appointment before refilling medications.
CPAP and BiPAP Supply Reorder
If you’re a current patient currently using a CPAP or BiPAP and have been seen recently in our office for a follow-up appointment, you may request PAP supplies by telephone. Please call (615) 893-4896 and provide the following information:
- Full name
- Date of birth
- PAP supplies needed
PLEASE NOTE: The Sleep Centers of Middle Tennessee reserves the right to require a follow-up appointment before ordering PAP supplies.
Insurance and Financial Billing Policy
As a service, we will complete and submit insurance forms to your insurance carrier. You will need to present your insurance card at your first visit and may need to verify this information at subsequent office visits. If there are any changes in your insurance and/or personal information (i.e. name, address, phone number), please notify our office so we can maintain accuracy in our billing. We are service providers for the following plans:
- Blue Cross/Blue Shield
- United Healthcare
- and others…
